Dialysis Technicians Initial Credential

​​​You must upload all DT documents to the Kentucky Board of Nursing Nurse Portal.

Apply for initial credentialing if you completed a DT Training Program in Kentucky:


Apply for initial credentialing if you completed a DT Training Program Out of State:​

  • Submit the application through the Kentucky Board of Nursing Nurse Portal. You will need to create an account through the nurse portal and then you will select Apply for a License located on the Nurse Portal Dashboard.​
  • Applications are valid for one year. If requirements are not met within one year, your application will expire and you will have to start the process over again
  • Pay application fee of $70 (ALL FEES ARE NON-REFUNDABLE)​
  • Complete the request for the ​ state and federal criminal ​background check through IdentoGO®​
  • Complete and submit the Checklist for Dialysis Technician Competency Validation​, which is located in the KBN Document Library.
  • Submit proof of certification from one of the following organizations:
    • The Board of Nephrology Examiners Nursing Technology (BONENT);
    • The Nephrology Nursing Certification Commission (NNCC); or
    • The National Association of Nephrology
    • ​Technicians/Technologists (NANT)
  • Complete the KBN Out-of-State DT Training Module. The module will be sent by email upon application review.
  • Complete the KBN Out-of-State DT Training Module Quiz. The quiz will be sent by email upon application review. You may return the completed quiz to:

Reporting Criminal Convictions

If you have any criminal convictions, they will need to be reported on your application. The exception to this is traffic misdemeanors, other than DUIs. For information about what qualifies as a criminal conviction please review the Mandatory Reporting of Criminal Convictions.


Reporting Action in another Jurisdiction

An applicant shall provide a certified copy of any disciplinary action taken on any professional or business license in another jurisdiction and a letter of explanation with each application (initial, renewal, reinstatement).​
