  • Please write down your Complaint Number system generated in the event there is an issue you will need for future reference.
  • Please include as much information as possible in the complaint and follow the directions to upload pertinent information into the complaint portal. Also, note that the Complaint Portal Form will time-out after 30 minutes of inactivity. If the Form times out, you can email KBNdisciplinealert@ky.gov to request the link to recommence the submission of the Complaint Portal Form. You will need to include your complaint number referenced above.​

Immunity from civil and criminal liability​

KRS 314.093 states, “Persons and agencies that file a complaint, provide sworn or written statements, or otherwise participate in an investigation or administrative proceeding instituted pursuant to this chapter shall have immunity from civil or criminal prosecution that is based upon such information unless the person or agency committed perjury, acted in bad faith, or acted with gross negligence, recklessness, or malicious purpose.”

Kentucky is a Mandatory Reporting State

  • Nurses: Kentucky is a Mandatory reporting state in which any person, including the licensee, that has knowledge that a violation of laws pertaining to nurses is required to report it to the Board pursuant to KRS 314.031(4).
  • Dialysis Technicians: Kentucky is a Mandatory reporting state in which any person, including the licensee, that has knowledge that a violation of laws pertaining to dialysis technicians is required to report it to the Board pursuant to KRS 314.035(3).
  • Licensed Certified Professional Midwives: A Licensed Certified Professional Midwife (LCPM) or an employer of an LCPM, must report any violation of KBN regulations by an LCPM to the Board KRS 314.408(4).
  • Any violations detailed in KRS 314.091(1), 201 KAR 20:470, and 201 KAR 20:630 are subject to these reporting requirements.


​What if I have a complaint against someone other than a nurse?​

If your complaint is against someone other than a licensed nurse, please refer the complaint to the appropriate state regulatory agency.


Student Complaints​

KBN accepts formal complaints from students and the general public where a program of nursing is believed to have violated Kentucky education law.
