Self-Report to KARE

​​​Self-reporting to the Board is required for admission into the Kentucky Alternative Recovery Effort (KARE) for Nurses Program. Per 201 KAR 20:450 Section 2, one must submit a written request for admission to the KARE Program and admit to having a substance use disorder. Please visit our KARE and Frequently Asked Questions pages for additional information regarding the KARE for Nurses Program.


If you wish to Self-report to the KARE for Nurses Program, please do so by submitting a KARE Self-Report in the Kentucky Complaint Portal.

  • Please write down your Complaint Number system generated in the event there is an issue you will need for future reference.

  • Select KARE – Self-Report from the drop down box and complete all applicable information.  *In the “Description of Incident" section, please include the following information:

    • Request for admission to the KARE for Nurses Program.

    • Admission to having a problem with substance use disorder.

    • Include a detailed history indicating what has transpired leading to this point.

  • Please note that the KARE – Self-Report/Complaint Portal Form will time-out after 30 minutes of inactivity. If the Form times out, you can write the Compliance Section to request the link to recommence the submission of the Complaint Portal Form. You will need to include your complaint number referenced above.
