If the dialysis technician credential has lapsed for less than one year
- Submit the application through the Kentucky Board of Nursing Nurse Portal. You will need to create an account through the nurse portal and then you will select Apply for Reinstatement located to the right of your credential number on the
Nurse Portal Dashboard.
- Pay application fee of $100 (ALL FEES ARE NON-REFUNDABLE)
- Applications are valid for one year. If requirements are not met within one year, your application will expire and you will have to start the process over again
- Submit proof of certification from one of the following organizations:
- The Board of Nephrology Examiners Nursing Technology (BONENT);
- The Nephrology Nursing Certification Commission (NNCC); or
- The National Association of Nephrology Technicians/Technologists (NANT).
- Complete the request for the
state and federal criminal background check through IdentoGO®
If the dialysis technician credential has lapsed for more than one year and you have not worked in another state:
- Submit the application through the Kentucky Board of Nursing Nurse Portal. You will need to create an account through the nurse portal and then you will select Apply for Reinstatement located to the right of your credential number on the Nurse Portal Dashboard.
- Pay application fee of $100 (ALL FEES ARE NON-REFUNDABLE)
- Applications are valid for one year. If requirements are not met within one year, your application will expire and you will have to start the process over again
- Submit proof of certification from one of the following organizations:
- The Board of Nephrology Examiners Nursing Technology (BONENT);
- The Nephrology Nursing Certification Commission (NNCC); or
- The National Association of Nephrology Technicians/Technologists (NANT).
- Complete the request for the state and federal criminal background check through IdentoGO®
- Complete a dialysis technician training program approved by the Board pursuant to the criteria established in the
Dialysis Technician Training Program Guide, which is located in the KBN Document Library. Submit proof of program completion of the program.
- Complete and submit the Checklist for
Dialysis Technician Competency Validation, which is located in the KBN Document Library.
If the dialysis technician credential has lapsed for more than one year and you have worked in another state:
- Submit the application through the Kentucky Board of Nursing Nurse Portal. You will need to create an account through the nurse portal and then you will select Apply for Reinstatement located to the right of your credential number on the Nurse Portal Dashboard.
- Pay application fee of $100 (ALL FEES ARE NON-REFUNDABLE)
- Applications are valid for one year. If requirements are not met within one year, your application will expire and you will have to start the process over again
- Submit proof of verification of working as a Dialysis Technician in another state
- Submit proof of certification from one of the following organizations:
- The Board of Nephrology Examiners Nursing Technology (BONENT);
- The Nephrology Nursing Certification Commission (NNCC); or
- The National Association of Nephrology Technicians/Technologists (NANT).
- Complete the request for the state and federal criminal background check through IdentoGO®
Reporting Criminal Convictions

Reporting Action in another Jurisdiction
An applicant shall provide a certified copy of any disciplinary action taken on any professional or business license in another jurisdiction and a letter of explanation with each application (initial, renewal, reinstatement).
