Apply for Initial Licensure
Compete the below list of application requirements for initial licensure.
Complete the application and submit the fee.
- Submit the application through the
Kentucky Board of Nursing Nurse Portal. You will need to create an account through the nurse portal and then you will select Apply for a License located on the Nurse Portal Dashboard.
- Applications are valid for one year. If requirements are not met within one year, your application will expire and you will have to start the process over again
- Pay the application fee of $500 (ALL FEES ARE NON-REFUNDABLE)

Required documentation
- Submit proof of the following while filling out the LCPM application through the Nurse Portal:
- Submit one of the following:
- Official transcript from MEAC accredited program*;
- Midwifery Bridge Certificate if certified by NARM prior to January 1, 2020; or
- Midwifery Bridge Certificate if licensed in another state that does not require an accredited education and proof of licensure in another state
- NARM Certification
- Proof of completion of the Domestic Violence course pursuant to KRS 194A.540
- Proof of completion of the Pediatric Abusive Head Trauma course pursuant to KRS 620.020(8)
- Neonatal Resuscitation Program (NRP) Certification
- American Heart Association Basic Life Support (BLS) Certification
- Proof of licensure in another state (if applicable)

*An official transcript is a transcript that is sent directly from the MEAC accredited program, through a clearing house to the KBN, or a copy obtained by the applicant that is in the original sealed, unopened envelope from the MEAC accredited program.
Transcripts can be sent electronically via a clearinghouse or the MEAC accredited program directly to the KBN to the email address: kbn.lcpmcred@ky.gov

How long does it take for a license to be issued?
- Licenses may be issued within 14 business days after all documents have been received and reviewed. However, if you respond "Yes" to the disciplinary or conviction questions it can take up to three months to process your application.
Reporting Criminal Convictions
- If you have any criminal convictions, they will need to be reported on your application. The exception to this is traffic misdemeanors, other than DUIs. For information about what qualifies as a criminal conviction please review the

Reporting Action in another Jurisdiction
- An applicant shall provide a certified copy of any disciplinary action taken on any professional or business license in another jurisdiction and a letter of explanation with each application (initial, renewal, reinstatement).
