Affidavit of Graduation (AOG) Portal




Prelicensure program of nursing administrators and designated staff will verify the education reported by exam applicants in the AOG Portal. Each user will only be able to review and approve reported education for the program(s) of nursing for which they have been designated as users. All AOG Portal users, including Program Administrators, must:

  • Be designated by the Program Administrator on the designation form for each program of nursing for which they will be granted access; and
  • Submit a completed Attestation Statement for each program of nursing for which they will be granted access.​

Document Submission






The Kentucky Board of Nursing is responsible for approving and regulating pre- and post-licensure nursing education programs, dialysis technician (DT) training programs, sexual assault nurse examiner (SANE) training programs, and licensed certified professional midwife (LCPM) training programs training programs, and certified medication aide (CMA) in the state of Kentucky. This oversight involves a substantial amount of documentation, reporting, and time spent by the programs’ faculty and staff as well as by Board staff.

To streamline and simplify these processes, KBN has leveraged EDvera, a workflow automation and document management platform that enables end-to-end management of complex application processing, audits, and reporting.

Staff from Kentucky regulated educational institutions and training programs must log into the EDvera website at kbn.edvera.com to submit all documentation to the Board. Access the EDvera platrform.

Institutions desiring to open a new nursing program must email KBN.Education@ky.gov stating the intent to open a program and requesting access to EDvera to submit the required documentation. Institutions desiring to open a new CE Providership, SANE Training Program, DT Training Program, or LCPM Training Program must email KBN.Practice@ky.gov stating the intent to open a program and requesting access to EDvera to submit the required documentation. Any documentation submitted via mail or email will have to be resubmitted in EDvera once access has been granted.

Opening a New Nursing Program



Pursuant to 201 KAR 20:280 Section 3(1), a governing institution desiring to establish a program of nursing shall submit to the Board a Letter of Intent and the non-refundable $2,000.00 fee required by 201 KAR 20:240. The fee must be remitted at the time that the Letter of Intent is submitted.

Pursuant to 201 KAR 20:280 Section 4(2), a prospective nursing program shall not be announced, advertised, or students admitted to the program of nursing until the proposal has been approved and developmental status has been granted by the Board. A program is not approved until the training program receives an official approval letter from KBN.

To gain access to EDvera to be able to submit the required Letter of Intent and fee, the governing institution’s President/CEO, the designated Prelicensure Program Administrator, the designated APRN Chief Nurse Administrator, the designated APRN Program Coordinator, or the individual responsible for completing and submitting the Letter of Intent (if the above individuals have not been designated) must submit a request for an EDvera account via the link in the “Request Access to EDvera” section below.

​Once KBN staff have set up the institution’s profile and created an EDvera user account for the individual(s) indicated in the request form, the individual(s) will receive an email with a link to finish the registration process and create a password for their account. After completing the registration process, the user(s) will have full access to the EDvera system for the prospective nursing program.
