To renew the DT credential you must:
- Submit the application through the Kentucky Board of Nursing Nurse Portal. You will need to create an account through the nurse portal and then you will select Apply for Renewal located to the right of your credential number on the Nurse Portal Dashboard.
- Pay application fee of $35 (ALL FEES ARE NON-REFUNDABLE)
- If the renewal application is not received prior to the expiration date, the DT must apply for reinstatement of the credential.
- Submit proof of certification from one of the following organizations:
- The Board of Nephrology Examiners Nursing Technology (BONENT);
- The Nephrology Nursing Certification Commission (NNCC); or
- The National Association of Nephrology Technicians/Technologists (NANT).
Reporting Criminal Convictions
If you have any criminal convictions, they will need to be reported on your application. The exception to this is traffic misdemeanors, other than DUIs. For information about what qualifies as a criminal conviction please review the

Reporting Action in another Jurisdiction
An applicant shall provide a certified copy of any disciplinary action taken on any professional or business license in another jurisdiction and a letter of explanation with each application (initial, renewal, reinstatement).
