Board Ordered Evaluations

​​I have been told to obtain a Board Ordered Evaluation. What do I need to do?

If the board has reasonable cause to believe that any licensee/credential holder, applicant for licensure/credentialing or holder of the privilege to practice as a nurse; credential holder; or holder of a temporary work permit is unable to practice with reasonable skill and safety or has abused alcohol or drugs, it may require the person to submit to an evaluation as listed below:

  • ​mental health
  • neuropsychological
  • psychosocial
  • psychosexual
  • substance use disorder
  • physical 

​The person ordered to obtain the evaluation shall have the evaluation submitted to the Board within thirty (30) days, unless due to circumstances beyond the person's control. The board may initiate an action for immediate temporary suspension pursuant to KRS 314.089 or deny the application until the person submits to the required evaluation.

In accordance with 201 KAR 20:163, the evaluation can only be completed by a licensed or certified practitioner designated by the board as listed below:

  1. ​​Psychiatrist;
  2. Physician Addictionologist;
  3. Psychologist;
  4. Advanced Practice Registered Nurse;
  5. Certified Alcohol and Drug Counselor;
  6. Certified Chemical Dependency Evaluator;
  7. Social Worker;
  8. Employee Assistance Program Specialist; or
  9. Marriage and Family Therapist.

The licensee/credential holder, applicant for licensure/credentialing or holder of the privilege to practice as a nurse; credential holder; or holder of a temporary work permit shall bear the cost of any evaluation ordered by the board.


Upon the failure of the person to submit the evaluation within thirty (30) days, unless due to circumstances beyond the person's control, the board may initiate an action for immediate temporary suspension pursuant to KRS 314.089 or deny the application until the person submits to the required evaluation.
