Please note the following:

  • There is not currently an online option available to update the mailing address or email address for LCPMs.
  • The above forms will say address change on the first screen but you will be able to use them to update both your mailing address and email address.
  • When you update your mailing address or email address online through one of the above forms the information in the KBN database will be updated immediately.
  • You must declare your primary state of residence if you are a nurse.
  • Do not use special characters, such as commas, periods, hyphens, slash marks, etc.

Change the name that is on file with the KBN by completing one of the forms with the following key words that are located in the KBN Document Library and completing the below instructions:

In addition to completing the form, you will need to do the following:

  • Send a check or money order in the amount of $25 to the address on the form (NON-REFUNDABLE)
    • During the renewal period, a name change can be submitted before or after a license is renewed, but the fee of $25.00 is still required.
    • Note: If you have submitted an application for licensure and have not been issued a KY license at the time you are requesting a name change, you DO NOT have to pay the $25 fee. You are required to complete the name change request form and fax it with the required documents to 502-429-3336 or mail it to the KBN at the address listed on the form.
    • If you submit the name change form with the fee and legal document(s) and you are in an application process, you will not be issued a refund.
  • Submit a copy of one of the required legal name change documents with the form:
  • Marriage certificate
  • Divorce decree showing the legal name change
  • Court order
  • Social security card
