Dialysis Technician (DT) Credential

Apply for initial credentialing if you completed a DT Training ​Program in Kentucky:

Apply for initial credentialing if you completed a DT Training Program Out of State:

  • Complete the Application for Dialysis Technician Credential located in the KBN Document Library and pay the application fee of $70 (ALL FEES ARE NON-REFUNDABLE).
  • Complete the request for the state and federal criminal background check through IdentoGO®
  • Complete and submit the Checklist for Dialysis Technician Competency Validation, which is located in the KBN Document Library.
  • Submit proof of certification from one of the following organizations:
    1. The Board of Nephrology Examiners Nursing Technology (BONENT);
    2. The Nephrology Nursing Certification Commission (NNCC); or
    3. The National Association of Nephrology
    4. Technicians/Technologists (NANT).
  • Complete the KBN Out-of-State DT Training Module. The module will be sent by email upon application review.
  • Complete the KBN Out-of-State DT Training Module Quiz. The quiz will be sent by email upon application review. You may return the completed quiz to:
  • Sign and Submit the KBN DT Attestation Form to:
  • If you have submitted an application for credential and have not been issued a KY credential at the time you are requesting a name change, you DO NOT have to pay the $25 fee.

  • You are required to complete the Name Change Request Form for Dialysis Technicians Only​ and send ​with the required legal documents to:
  • Submit a copy of one of the required legal name change documents with the name change form:
    • Marriage certificate
    • Divorce decree showing the legal name change
    • Court order
    • Social security card​

Note: If you submit the name change form with the fee and legal document(s) and you are in an application process, you will not be issued a refund.​

Dialysis Technicians renew annually. The renewal fee is $35. (ALL FEES ARE NON-REFUNDABLE).

KBN will mail an application form for renewal of the DT credential at least two months prior to the expiration of the DT credential. If the DT does not receive the credential renewal application, they should contact the KBN.

If the renewal application is not received prior to the expiration date, the DT must apply for reinstatement of the credential.

Renewal application and renewal fee of $35​ (ALL FEES ARE NON-REFUNDABLE).

To renew the DT credential you must submit:

  • Submit the DT renewal application and the application fee of $35 (ALL FEES ARE NON-REFUNDABLE).
  • Submit proof of certification from one of the following organizations:
    1. The Board of Nephrology Examiners Nursing Technology (BONENT);
    2. The Nephrology Nursing Certification Commission (NNCC); or
    3. The National Association of Nephrology Technicians/Technologists (NANT).

Note: It is the responsibility of each DT to maintain his/her current address with the Board office. A fee of $25 will be charged for a duplicate renewal application that is requested due to the failure to maintain a current mailing address. (ALL FEES ARE NON-REFUNDABLE). If the dialysis technician credential has lapsed for less than one year

  • Submit the Application for Dialysis Technician Credential located in the KBN Document Library and pay the application fee of $100 (ALL FEES ARE NON-REFUNDABLE).
  • Submit proof of certification from one of the following organizations:
    1. The Board of Nephrology Examiners Nursing Technology (BONENT);
    2. The Nephrology Nursing Certification Commission (NNCC); or
    3. The National Association of Nephrology Technicians/Technologists (NANT).
  • Complete the request for the state and federal criminal background check through IdentoGO®

If the dialysis technician credential has lapsed for more than one year and you have not worked in another state:

  • Submit the Application for Dialysis Technician Credential located in the KBN Document Library and pay the application fee of $100 (ALL FEES ARE NON-REFUNDABLE).
  • Submit proof of verification of working as a Dialysis Technician in another state
  • Submit proof of certification from one of the following organizations:
    1. The Board of Nephrology Examiners Nursing Technology (BONENT);
    2. The Nephrology Nursing Certification Commission (NNCC); or
    3. The National Association of Nephrology Technicians/Technologists (NANT).
  • Complete the request for the state and federal criminal background check through IdentoGO®

Credentials may be issued within 14 business days after all documents have been received and reviewed. However, if you respond "Yes" to the disciplinary or conviction questions it can take up to three months to process your application.
