Dialysis Technician (DT) Credential

Initial Credential Requirements

Complete the application and submit the fee.

Complete the Application for Dialysis Technician Credential located in the KBN Document Library and pay the application fee of $70 (ALL FEES ARE NON-REFUNDABLE).

The Application for Dialysis Technician Credential submitted for initial credentialing shall be valid for six months from the date of receipt by the board.

Submit information regarding training program

Submit a copy of the DT Training Program Completion Certificate as proof of completion of the DT Training Program Out-of- State Training Programs: If you completed a dialysis technician training program outside of Kentucky, you will also need to submit documentation of the DT training program content and complete additional Kentucky specific regulatory requirements, including the requirements outlined in the DT Training Program Guide, which is located in the KBN Document Library.

Individuals who complete an out-of-state training program must complete the Checklist for Dialysis Technician Competency Validation, which is located in the KBN Document Library.

Submit documentation regarding a name change, if applicable

If you have submitted an application for credential and have not been issued a KY credential at the time you are requesting a name change, you DO NOT have to pay the $25 fee. You are required to complete the name change request form and fax it with the required documents to 502-429-3336 or mail it to the KBN at the address listed on the form. A copy of the form can be located in the KBN Document Library using the search terms Name Change. Note: If you submit the name change form with the fee and legal document(s) and you are in an application process, you will not be issued a refund.

Submit a copy of one of the required legal name change documents with the form:
  • Marriage certificate
  • Divorce decree showing the legal name change
  • Court order
  • Social security card

The credential will be issued for a two-year (24-month) period from the date it is originally issued.

Renewal of Dialysis Technician Credential

The renewal fee is $70. (ALL FEES ARE NON-REFUNDABLE).

KBN will mail an application form for renewal of the DT credential at least two months prior to the expiration of the DT credential. If the DT does not receive the credential renewal packet, they should contact the KBN.

Each DT must apply for renewal of his/her credential at least one (1) month prior to its expiration date. Failure to renew one month prior to the expiration of the DT Credential will result in a $25 fee in addition to the $70 renewal fee. (ALL FEES ARE NON-REFUNDABLE) To renew the dialysis technician credential, the following must be submitted to the KBN:

  • Renewal application and renewal fee of $70 (ALL FEES ARE NON-REFUNDABLE).
  • Submit evidence of obtaining certification from one of the nationally recognized certification programs. Obtain more information about national certification requirements.

Note: It is the responsibility of each DT to maintain his/her current address with the Board office. A DT Change of Address form is available in the KBN Document library using a keyword search of ​ Address/Email Change Form for Dialysis Technicians. A fee of $25 will be charged for a duplicate application form that is issued due to failure to maintain a current mailing address. (ALL FEES ARE NON-REFUNDABLE).

The credential will be renewed for a two-year (24-month) period from the date it is originally issued.

Reinstatement of Lapsed Dialysis Technician Credential

If the dialysis technician credential has lapsed for less than one year

Complete the Application for Dialysis Technician Credential located in the KBN Document Library and pay the application fee of $100 (ALL FEES ARE NON-REFUNDABLE).

The Application for Dialysis Technician Credential submitted for reinstatement shall be valid for six months from the date of receipt by the board. Complete the state and federal criminal background check through IdentoGO®

If the dialysis technician credential has lapsed for more than one year

Complete the Application for Dialysis Technician Credential located in the KBN Document Library and pay the application fee of $100 (ALL FEES ARE NON-REFUNDABLE).

Complete the state and federal criminal background check through IdentoGO®

Complete a dialysis technician training program approved by the Board pursuant to the criteria established in the Dialysis Technician Training Program Guide, which is located in the KBN Document Library.

Complete the Checklist for Dialysis Technician Competency Validation, which is located in the KBN Document Library.


Reporting Criminal Convictions

If you have any criminal convictions they will need to be reported on your application. The exception to this is traffic misdemeanors, other than DUIs. When Can I Begin Employment

A Dialysis Technician Applicant (DTA) or Dialysis Technician Credential (DTC) must be issued before beginning employment.

When will my DT credential be issued?

Credentials may be issued within 14 business days after all documents have been received and reviewed. However, if you respond "Yes" to the disciplinary or conviction questions it can take up to three months to process your application.
