Instructions on how to request a certificate suitable for framing
Complete the Certificate of Licensure Order Form which is also located in the KBN Document Library to obtain a certificate (8 1/2" x 11") suitable for framing.
The cost is $30 per certificate to be paid by check or money order payable to the Kentucky Board of Nursing or through credit card via prompt pay.
The following certificates are available:
- LCPM
- LPN
- RN
- RN and APRN
- APRN only
- RN and SANE
- SANE only
The selection(s) that you choose on the order form will be printed on the certificate.
If you hold an RN license and an APRN license or SANE credential, both of these will be reflected on one certificate.
The mailing address is provided on the order form. Please allow 4 to 6 weeks for delivery.
Certificates are not proof of licensure.
