Universal Applications
The universal application for licensure is for both the licensed practical nurse and the registered nurse. You can now submit an electronic universal application for licensure by:

Submitting the application is only one step in the licensure process. Payment information is provided below. For complete information on each type of licensure, go to Credentials and Licensure Information and select the licensure type you are interested in.

Fees:
- $120 — Examination
- $150 — Endorsement
- $120 — Reinstatement
Methods of Electronic Payment:
- Credit or debit MasterCard or Visa
- Personal or business checking or savings account (see sample of routing number and account number below)
- A prepaid credit card, available from most banks
