Licensure By Reinstatement
The licensure by reinstatement application process allows a person to reinstate a lapsed license to an active status. Downloadable forms and instructions are included here.

Application Requirements |

KBN uses one application form for several licensure processes. On the first page of both the application form and the instructions, the application methods are listed, along with the sections that you must complete for each method type. Refer to the sections as you complete the application form.
Please note that there is a separate application form if you are an ARNP. Go to the Advanced Registered Nurse Practitioner Credential page.

RN or LPN Reinstatement |

RN or LPN reinstatement applicants must submit the following:
- Completed Reinstatement Application Form [PDF Format - 204k] (includes instructions and attachments)
- $120 fee
- Copy of active nursing license in another state or U.S. jurisdiction, with signature if required
- Proof of meeting the Reinstatement Competency Requirement (Note: A list of CE providers is included when you access the requirement link)

Criminal History Reporting |
