Foreign Educated Applicants Seeking Initial Licensure By Examination

Foreign graduates seeking licensure in Kentucky who have not been licensed in the U.S. or its jurisdictions must:

KBN Application Requirements

KBN uses one application form for several licensure processes.  On the first page of both the application form and the instructions, the application methods are listed, along with the sections that you must complete for each method type.  Please refer to the sections as you complete the application form.

All examination applicants must submit the following:

  • Completed Examination Application Form [PDF Format - 71k] (includes instructions and attachment)
  • $110 fee [Fees are subject to change and non-refundable]
  • Completed Criminal Background Request Form (also known as CourtNet - this form is included with the application download) [PDF Format - 13k]

Registration for NCLEX

Method of registration:

  • Website:  VUE
  • Telephone:  1-866-496-2539
  • Mail:  NLCEX, PO Box 64950, St Paul, MN, 55164-0950

Registration by web or telephone:

  • Requires a valid credit card
  • Results in faster processing time, when an email address is provided 
  • ATT emailed directly to you

Registration by mail:

  • Your ATT will be mailed to you by USPS and may take up to 4 weeks to be received
NCLEX Eligibility and Authorization To Test (ATT)

After review of your KBN application, the VisaScreen  certificate, and the criminal history report from AOC, you will be made eligible to sit for the NCLEX examination.  After being made eligible to test, you will receive an Authorization to Test (ATT) from the testing service.

If you registered via the Internet or by telephone and provided a valid email address, your ATT will be sent to your email address within 3 business days of the date KBN makes you eligible to test.  If you register by mail, it may take up to 4 weeks to receive your ATT.  If you lose your ATT, you must contact the testing service, not KBN. 

After you receive your ATT, you may contact Pearson/VUE to schedule a test date.

Receiving Your Permanent Kentucky License

After KBN receives notification from Pearson/VUE that you passed the NCLEX examination, you are eligible for a Kentucky nursing license.  BEFORE a nursing license will be issued, you must present a copy of a valid social security card to KBN.
