Foreign Educated - Licensure by Endorsement in Another U.S. Jurisdiction

KBN uses one application form for several licensure processes.  On the first page of both the application form and the instructions, the application methods are listed, along with the sections that you must complete for each method type.  Please refer to the sections as you complete the application form.

Foreign educated nurses obtaining a Kentucky nursing license by endorsement must submit the following:

• Completed Endorsement Application Form [PDF Format - 80k] (includes instructions and attachment)
• $150 fee
• Copy of active nursing license in another state or U.S. jurisdiction, with signature if required
• Copy of a VisaScreen Certificate issued by the Commission on Graduates of Foreign Nursing Schools (CGFNS)
• Proof of meeting the Endorsement Competency (CE) Requirement, including AIDS/HIV education
• Verification of Original Licensure from another state or U.S. jurisdiction 
• Fingerprint card with $19.50 fee
• Maintain proof of legal residency in the U.S.A.
