Criminal Background Report (CourtNet)
All applicants for licensure by examination are required to submit a Criminal Background Request Form (CourtNet) [PDF Format - 13k] from the Administrative Office of the Courts (AOC).
To obtain a criminal history report, you can request one from the AOC website with payment by credit card at http://courts.ky.gov and click on Obtain a Criminal History Report. If you apply online to the AOC, the KBN will be able to retrieve the report from them.
Another way to obtain a criminal history report is to complete the Criminal Background Request Form (CourtNet) [PDF Format - 13k] and submit it as directed on the form. The AOC will send a copy of the results to the applicant, as well as mail a copy to the KBN.
If you apply to KBN using a paper application, you must mail a copy of the criminal history report you receive from the Administrative Office of the Courts with your application for licensure.
A criminal history report in every surname you ever had is required.
