Criminal Background Report (CourtNet)
You must submit a criminal background report with your application for licensure.
To obtain a criminal history report, complete the Criminal Background Request Form (CourtNet) [PDF Format - 13k], and submit it as directed on the form. Note: This form is included when you download the Examination Application Form [PDF Format - 85k].
Your application will not be processed, and you will not be issued a Provisional License until the criminal history report(s) is received by KBN.
A criminal history report is needed for every name you have used, including your maiden name and any/all married names.
