Kentucky Unbridled Spirit (TM)

Verification of Original Licensure

All applicants must submit verification of licensure as an RN from their original state of licensure.

You must either:

  • Complete the verification process through Nursys™ External Link - You are now leaving the .gov domain. (if your state of original licensure is listed on that website), or
  • Complete the top portion of the Verification of Licensure Form (Attachment 2) [PDF Format - 136k] and mail it with the fee to your state of original licensure.
