Address, Email, or Name Change
When requesting a change of address, email, and/or name change you must include a sentence declaring your primary state of residence and whether or not you practice ONLY in a military/federal facility. Failure to provide this information will result in your request being returned to you.

If you hold a Kentucky nursing license, you have two options for changing your address or email on record with KBN:
- Online Address/Email Change
The KBN record will be updated as soon as you submit your address or email change information.
Note: When changing your address online, please do NOT use any special characters in the address change, such as commas, periods, hyphens, slash marks, or number signs, etc. Online address changes are to be inserted on the first line of the address section; only use line 2 of the address change when needed.
- Request an address or email change in writing, by printing the Address/Email Change Form [PDF Format - 14k]
Complete the form and mail it to the KBN address provided on the form. It may take up to 3 weeks for the change to become effective.
For security purposes, KBN will not accept an email request to change an address.

Name changes cannot be done online.
- Request a name change in writing, by printing the Name Change Form [PDF Format - 17 KB]
Complete the form and mail it, along with the appropriate attachments, to the KBN address provided on the form.
Required attachments include:
- $35 processing fee
- Your current active license card
- Required legal documentation (include a copy of any ONE of the following)
- Certificate of Marriage
- Divorce Decree Showing the Legal Authority for a Name Change
- Court Order
- Social Security Card
NOTE: Requests may be submitted at any time. However, it is strongly recommended that you submit the request for a name change at least 3 weeks prior to renewing your Kentucky nursing license.
