Verification of Original Kentucky Licensure

Verification of original Kentucky RN or LPN licensure data to another Board of Nursing must be completed through Nursys™ .

To verify APRN licensure, after completing the Nursys™  verification request for RN license, contact the APRN Program Coordinator. 

To verify original Kentucky licensure to any entity other than a board of nursing, complete the request form [PDF Format - 24KB] and return it with a check or money order in the amount of $50 to KBN.
