Ordering KNAR Database Roster
To obtain addresses for state registered nurse aides, complete the KNAR Data Roster Download Request Form [PDF Format - 33k], and return it with the $480 base fee to the KBN address provided on the form. The base fee must be paid in advance with a check or money order payable to the Kentucky Board of Nursing.
Orders will be sent via email unless otherwise indicated. If you prefer another method of shipment, you must indicate which method on the bottom of the order form.
APPROXIMATELY 4 to 6 WEEKS MAY BE NEEDED TO PROCESS THE REQUEST CONTINGENT UPON COMPUTER TIME AVAILABILITY.
