Dialysis Technician Applicant Status (DTA)
Kentucky may issue a one-time only, Dialysis Technician Applicant status (DTA) to credentialing applicants when certain regulatory requirements are met. A DTA allows the applicant to practice dialysis in Kentucky while awaiting a permanent Kentucky Dialysis Technician Credential.

Q: Who is eligible for a dialysis technician applicant (DTA)? A: Applicants for credentialing who have completed an approved Kentucky Dialysis Technician Training Program are eligible for a DTA.
Q: What are the requirements to receive a DTA? A: A Dialysis Technician Applicant status will be issued when the following information is received:

A completed dialysis technician application form

Copy of DT Training Program Certificate

Completed fingerprint card

Fee of $70 (initial) or $100 (reinstatement) application processing fee

FBI Fingerprint Card must be submitted directly to the Kentucky State Police, Records Branch, 1266 Louisville Rd, Frankfort, KY 40601, with the $16.50 processing fee.

Criminal history report must: (if you did not request AOC to submit to the Board electronically)

Be from the Kentucky Administrative Office of the Court

Q: How long does it take to receive a DTA? A: When all requirements are met, it may take up to 14 days from the date KBN receives your application and information above for a DTA to be issued.
Q: When does a DTA expire? A: A DTA is valid for 6 months from the date of issue. It cannot be extended, and you may only be issued a DTA once.
Q: How does a DTA affect my ability to practice dialysis? A: You may practice within the full scope and practice of a dialysis technician with a DTA.
Q: When will I be issued a permanent credential? A: Within 14 days of receipt of a criminal history report from the FBI, a credential will be issued.
Q: What happens if my DTA expires and I am not credentialed by KBN? A: If your DTA expires and you do not hold a Kentucky dialysis technician credential, you must reapply to Kentucky for a credential, pay the application fee, and cease working as a dialysis technician in Kentucky until all requirements are met and your permanent credential is issued.

The DTA CANNOT be extended, and you CANNOT be issued a second one.
