Licensed Louisiana Nurses Displaced Due to Hurricane Katrina
Any licensed nurse from the state of Louisiana who is displaced due to Hurricane Katrina, and is, or will be, residing in the Commonwealth of Kentucky and seeking registration or licensure in Kentucky shall:
- Complete an application for a Kentucky license by endorsement [see Endorsement page].
- Submit a copy of a current, active Louisiana nurse’s license.
- Submit an application fee of $120:
- Applications received on or after December 1, 2005 will be reviewed on a case-by-case basis if the applicant indicates an inability to pay the fee due to situations caused by Hurricane Katrina.
- KBN will access NurSys to determine whether any discipline has been attached to the applicant’s nursing license(s).
- A fingerprint card and the $24 fingerprint processing fee must be received before a temporary work permit will be issued.
