Lost or Stolen Licenses/Registrations
Effective May 27, 2002, KBN began capturing data on nursing licenses and registrations that have been reported lost or stolen. A duplicate license or registration has been issued for these individuals.
Please note that no action has been taken on these licenses as a result of the duplication process.

How Do I Report a Stolen License? |

If you have had your nursing license card lost or stolen, report it immediately to KBN using the I&D Contact Form.

Complete the Duplicate Request Form [PDF Format - 16k] and submit it along with the appropriate attachments and the $35 fee to the KBN address provided on the form.
