Certificates for Framing

Complete the Certificate of Licensure/Registration Order Form [PDF Format - 32k] to obtain a certificate (8 1/2" x 11") suitable for framing.  The cost is $30 per certificate to be paid by check or money order, payable to the Kentucky Board of Nursing.

If you hold a duel RN license and ARNP registration, both of these will be reflected on one certificate.

The mailing address is provided on the order form.  Please allow 6 to 8 weeks for delivery.
