Licensure By Reinstatement

All online applications will be unavailable on Friday May 18 from 4:30 pm until 7:30 pm.

Apply for license by reinstatement if your Kentucky license has expired.

Applications and documents are reviewed within 14 days.  However if you respond "Yes" to the disciplinary or conviction questions it can take up to 3 months for processing your application.

RN or LPN Reinstatement

All fingerprint cards and fees received at KBN will be returned to the applicant.  Initial fingerprint card and fee of $16.50 must be mailed to the Kentucky State Police at:

KBN will receive an electronic copy of the report, if you request the report online and follow the instructions accurately [PDF Format - 187KB].

  • Complete the Kentucky Jurisprudence Exam - Must be completed if your Kentucky RN or LPN license has been lapsed for one year or more.

Validate the expiration date of your license.

Request a paper application [PDF Format - 93KB] for a fee of $40.  The $40 paper application request fee is in addition to the application fee.
